60-120 = MOF
>120 = RIP.
Phases
0-6h - D+V+N+GI bleed
6-12 h - Recovery
12-48 hr - MOF, shock, acidosis
2-5 d - fulminant liver failure
2-6 wks Strictures and cirrhosis.
Serum Iron levels
VBG
Usually visible on CXR (ie: if ingestion unsure etc.)

SPECIFIC ANTIDOTE
Deferoxamine (Desferrioxamine) - Level >60 bad, >90 toxic.
Endoscopic removal of bezoar
Whole bowel irrigation if >60 mg/kg
Charcoal does nothing.
- Desferrioxamine i.v.i. 15-40 mg/kg/hr
- RCH – 15mg/kg/h, The rate is reduced after four hours so that the total intravenous dose does not exceed 80 mg/kg/24 hours
Endoscopic removal of bezoar
Whole bowel irrigation if >60 mg/kg
Charcoal does nothing.
