ONCOLOGY PATIENTS
Tazocin 100 mg/kg (piperacilin component)
  • Amikacin 22.5mg/kg + Vancomycin 15mg/kg QID if severely unwell.
Surgical triples:
Amp 50 mg/kg + Gent 5 mg/kg + Metro 12.5 mg/kg
